Extra Midi in & Out issues

I think I fried my first extra Midi in & out kit from smash (Well, The Pic got mighty hot, then it didn't get hot no more)

I've now got an extra one, and am wondering if anyone else has installed one of these? I'm sure there are people out there that have installed one with out probs..

It all seems pretty simple.. but I obviously screwed it up.

I also can't seem to find on the schems what the polarity of the 3 LED's on the kit are.. The Smash site does not seem to detail this.. (Please correct me if I'm wrong)
