Yeah there is a small problem with the file. Remove the blank lines. Then it will be fine.
Open it in notepad++ and use Edit>Line Operations> Remove empty lines. Or if you are handy with linux/osx then awk is your friend.

I would love to give more suggestions. However i have to give credit to some people complaining about off-site docu being bad. Wilba's mb6582.org domain seems to be offline. Leaving all the documentation for mb6582, sammichsid and sammichfm unavailable :sad: .
Looking at the datasheet for the YAC512 it has a Max supply voltage of 7V. However the OPA4348 has a Max supply voltage of 5.5V. So my main suspect would be the OPA4348. It could very well be that it has been damaged. Replacing it won't be easy as it is a SMD component. If anyone has some other suggestions be my guest.

Looks pretty good. Must admit i had some mishaps by placing stuff to close together in Eagle. The only thing i am worried about in KiCad is the libraries. There is quite a few for Eagle. Not sure how that is for KiCad?

You have the 6582 SID in there. Those don't run so hot. But just to be sure, you could put on some heatsinks.

These are 4mm shafts. The 6mm ones don't fit my panel.

yep, mono pots are your problem. They're supposed to be dual like this:http://www.mouser.com/catalog/specsheets/TW-700150.pdf
Are you sure these are the correct ones? I thought the MB6582 required pots with a 4mm shaft. Or is that just on my backpanel?

I can add the altered eps file to the wiki. So people are able to use the standard switches.

Besides cutting a few corners here and there i cannot see something obviously wrong with this build. The only modifications i see are in the power section and in the lcd connector.

If you are sure about this, then i think you should open the box up. Then post a highres photo of the top and bottom side of the mainboard.

The title is probably embedded in the file. I just dont have any clue how to change this in Logic.

I am not aware of any Mutable Instr. projects using the LT1026 (he does use the LT1054 a lot). Looking at the datasheet of the LT1026 with 5 Vin you will get to max 6,5-7V out. So that would not be feasible.
The simplest (and yet most expensive options) are the Traco TEN3 and the Recom RS3 units. They can convert 5V to +/-12V and still deliver 125mA.
@NorthernLightX, For audiophile powersupply options, the LM317 is a better option then the LM78xx series. But there are better options available.

I am currently constructing a PSU for my MB6582 unit. The PSU will consist of a Meanwell RPT60 with a LM2596 circuit to make the required 9 Volts. I already have a DIN7 connector, just need a proper cable to go from the PSU to the MB6582.
I feel a bit silly, but i am unsure what kind of cable to use for this. I could use a 4 core 230Volt cable for this, but that would be overkill and also would be way to thick. However i don't seem to be able to find anything that fits the bill. Any suggestions would be helpfull.

IC3 is the 78L05 power regulator. If you measure 6V on the output leg (the leg nearest to the power connector) then there is something wrong. I would double check all the components in that area for correct placement (C3, C5, IC3, X1)
If everything is placed correct and you say you have seen magic sparkles, then you probably short circuited something. My recommendation is to replace the 78L05.

Ok, maybe i will give it a go. I have a project where the board exceeds the size of which is allowed by the freeware version of Eagle. Didn't feel like paying 140 euro for the amateur version which then only allows a full euro card.
